Inspector comments: No points deducted. Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). Observed disinfectant wipes stored accessible to children in 134. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Observed stained ceiling tiles in Storage F closet. Check for leaks and repair/replace.

Inspector comments: #4. Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). Observed exterior of stall fixture soiled in girls bathroom in common bathroom that is nott being used at this time. Place out of place ceiling tile back into place in girls bathroom. #5.Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b). Observed spring loaded faucet fixture not properly working(short run time, less than 3second) on (1) handwashing sink in main boys bathroom. Have spring loaded faucet repaired on hand washing sink. Spring loaded faucet should operate for 15 seconds. #6. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Continue to wipe/clean walls throughout in hallway and classroom area. Continue to touch-up paint throughout in classroom area. Observed water stained ceiling tile in 123. Replace water stained ceiling tile and check roof for any leaks and HVAC system for condensation collecting on ventilation. Provide door sweep on base of door gym leading to exterior of building to eliminate gap along base of door. #7. All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411. Observed storage on floor in science storage room. No storage on the floor. Provide additional shelving in storage room and store supplies on shelving. #8. All windows and fixtures (grills, vents, blinds, drapes, lighting fixtures, etc.) shall be kept clean and in good repair. 15A NCAC 18A .2412 (b). Oberved return air ventilation in gyms and hallway area dusty. Clean ventilation in gym and hallway area. #11.There shall be no fly or mosquito breeding places, rodent harborages, or undrained areas on the premises. 15A NCAC 18A .2415 (a). Observed standing water in container within dumpster corral with mosquito larvae present. Container emptied at time of inspection. Store items inverted down so they do not collect or hold water.
