Inspector comments: Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). Observed urinal broken in boys cafeteria bathroom. Urinal has been blocked and covered to be put out of use until repaired.

Observed aerosols and disinfectant wipes stored accessible to children in several classrooms. Needs to be stored to avoid health hazards as directed on manufacturer label.

Inspector comments: Note: An ice machine was added in the nurse’s room (left of front desk) after CO. Ice machine not indirectly drained but tied into a pump which pushes the water to the sink discharge plumbing. Staff said the breaker keeps tripping and then there is water on the floor. I observed water pouring onto the floor from the pump, did not hear pump operating. Staff turned off ice machine until problem is fixed.
Note: vinyl wrapped ceiling tiles missed on approved plans

Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b). Observed acoustic tiles in kindergarten classrooms. Tiles need to be vinyl wrapped.
