Replace missing floor tiles around floor drain in upstairs shower in 6900B and 5300A shower. Repair floor near entrance to common toilet in 4200. Observed floor damaged in cell 13 in 1900. Observed shower tiles with mold in grout in 3900.All floors must be easily cleanable and in good repair. 15A NCAC 18A .1507(a).

Observed water stained ceiling tiles in hallway area on 6th floor and 5th floor. Repair any roof leaks and replace water stained ceiling tiles. Touch-up paint throughout in cells, showers and bathrooms. Thoroughly walls in showers in 4200 pod and repaint if needed. Repaint metal pole for showers and bathrooms stall doors throughout in pods where needed. Clean ceiling in shower in 4500A. Observed wall damage next to toilet in 1900 bathroom stall. Observed walls not painted around toilets in 1700 staff and inmate. Observed cell 23 wall paint damaged in 3625 Keep all walls and ceilings clean and in good repair. 15A NCAC 18A .1508 (a).

Clean return air ventilation in 4200 pod near upstairs shower. Clean ventilation covers in cells. Observed dusty vents in 3300 general area. Observed cell 23 with toilet paper covering vents in 3625.
Keep all ventilation equipment clean and in good repair. 15A NCAC 18A .1509 (b).
