Notes:
Pump housing changed since last inspection. 1.65 THP motor on pump with no placard on pump.
Only labeling on housing is P/N for housing 357149 (housing model number). No model on impeller.
PDSC form shows 1 hp Whisperflo. Picture of motor provided with no service factor listed US Motors. Email from contractor who replaced housing
stated impeller was not changed. Flow meter only reading 55 gpm.

Floors in restrooms have been changed to slope to drain, verified with level and contractor verified slip resistant, same product as other flooring. All sinks working at time of inspection.
Operating permit will be made available online and EH holds released. Log book provided.
