Newer records missing date of birth and address at time of inspection. Records must include name, date of birth, address, phone number and signature. Artist edited and printed new forms for correction. Will make additional visit to shop to ensure all artists are in compliance.
#6 · Tattoo room: furniture easily cleanable, kept clean
Two tattoo chairs observed torn, Styrofoam exposed on one and duct tape peelings. Furniture must remain smooth, easily cleanable and non-absorbent. Artist is removing these chairs and will use another station/furniture in good repair for tattooing. Corrected.

Trash outside of establishment starting to accumulate and creating insect/fly harborage area. Keep premises free of litter accumulating and fly, mosquito or other harborage area. Artist cleaning up area and speaking with other shops to maintain area/premises clean.
